For New Patients

There are many visual skills critical for reading and learning. Often, a vision problem is misdiagnosed as an attention problem or a learning disability due to similarity of symptoms. Anyone who is struggling with reading, has poor reading comprehension, or just isn't performing to their full potential, should consider scheduling a developmental vision evaluation. To help determine if you or your child should be tested, Do the Checklist.

Appointments

In addition to his specialty testing and treatment for Binocular Vision Disorders, Dr. Kotlicky provides comprehensive eye care for the entire family.  The checklist link above will determine if you need a Developmental Vision Evaluation.  If you just need your prescription updated or want a contact lens evaluation, a routine eye exam is sufficient.  If you are unsure of which type of appointment you need, our receptionist will be happy to assist you.  To schedule an appointment, please call us at (410) 730-5808 or use our online Request an Appointment form.

Insurance Information

We work with both major medical Insurance plans and vision plans.  Our specialty services (The Developmental Vision Evaluation and Vision Therapy) are covered under major medical Insurance, so it doesn't matter if you have a vision plan or not.  Dr. Kotlicky participates with Aetna, BCBS, BlueChoice, Carefirst and BCBS Federal as an in-network provider.  It is the patient's responsibility to check their policy to determine if their major medical insurance requires referrals or pre-certifications for specialists.  If insurance does not cover the initial evaluation and/or vision therapy, we will provide you with the necessary paperwork so you may file with your carrier for reimbursement.  We do not participate with any vision plans for routine eyecare or materials.  Patients pay at time of service and we provide paperwork for you to file with your plan for direct reimbursement.
